- Form fields include Account Number, Customer Name
- Selection to choose account status to become “Inactive” or “Deactivated”
- Details and notices displayed describing each status
- NOTE: quote includes full initial set up onto New Fluent Forms platform for broader support of certain features and requests
- Includes minor adjustments or updates as normal – while additional requests outside the initial scope may require more time
- Fluent Forms license is not included in the price and will be charged separately
